
Player/Participant Agreement
1. Agreement to Participate
I, the Parent/Guardian of the named Participant, agree to the Participant taking part in the Academy’s training, matches, camps and related activities. I confirm that the information I have provided in registration (including medical details and emergency contacts) is true and complete.

2. Acknowledgement of Risk & Liability Waiver
2.1 I acknowledge that participation in football/sport involves inherent risks of injury, illness and other harm. I accept and assume those risks on behalf of the Participant.
2.2 I confirm the Participant is physically able to participate and that I will notify the Academy promptly of any change to medical conditions or medications.
2.3 To the fullest extent permitted by law, I release and indemnify the Academy and its officers, employees, contractors, coaches, volunteers and agents (“Released Parties”) from any claim for injury, loss or damage arising out of the Participant’s attendance or participation, except where caused by the gross negligence or willful misconduct of a Released Party. (This clause is based on standard participant agreements used in Australian clubs and programs; it does not remove statutory rights that cannot be lawfully excluded.)

3. Medical Treatment & Emergency Care
3.1 In the event of an emergency, I authorise the Academy and its staff to seek medical assistance and arrange transport to hospital as necessary. I will be responsible for any medical or ambulance costs incurred.
3.2 The Academy will follow the Participant’s medical management plans (e.g., asthma, anaphylaxis) where provided in writing on registration.

4. Refund & Fee Policy
4.1 Deposit / Acceptance — Payment of fees and/or deposit secures the Participant’s place. Some components (e.g., non-refundable administration fee, capitation to governing bodies) may not be refundable.
4.2 Withdrawal by Parent/Guardian — If you withdraw before the program start date, you may be eligible for a partial refund less any non-refundable items; withdrawal after the program starts generally attracts no refund for the remainder of the program, unless otherwise agreed.
4.3 Medical Withdrawal — Where a Participant withdraws due to verified medical incapacity prior to the program start or early in the program, the Academy may offer a pro-rata refund or credit subject to provision of a medical certificate and deduction of non-refundable items.
4.4 Exceptional Circumstances — The Academy will consider requests for refunds in exceptional circumstances (e.g., serious illness, relocation) on a case-by-case basis.

5. Insurance
5.1 The Academy holds public liability insurance and (where applicable) personal accident cover for participants while engaged in Academy-run activities. This insurance may be limited in scope; parents/guardians are recommended to maintain their own private accident/health insurance for the Participant to cover medical costs, ambulance and other expenses.
5.2 Insurance does not replace the need for parents to ensure their child’s health and fitness for participation.

6. Privacy & Use of Personal Information
6.1 The Academy collects and holds personal information about the Participant and parent/guardian for the purposes of participation, administration, emergency contact, insurance, and communication about Academy activities. The Academy will handle this information in accordance with applicable privacy laws and its Privacy Policy.
6.2 Personal information will not be shared with third parties except for legitimate purposes (e.g., registration with governing bodies, medical care, authorised media use, or where required by law). You may request access to or correction of your personal information by contacting the Academy.

7. Photography, Video & Likeness Release
7.1 While participating in Academy activities, photographs, video or audio recordings may be taken. These recordings may be used by the Academy for promotional, educational, marketing, and archival purposes (including on the website and social media), unless the Academy is notified in writing of a restriction.
7.2 By signing, you grant the Academy a royalty-free, perpetual, worldwide licence to use images and recordings in which the Participant appears, in whole or part, for the Academy’s legitimate promotional purposes, unless you have indicated a photo opt-out on the registration form.
7.3 The Academy is committed to child-safe photography practices: images will not include personal contact details, identifying information, or images likely to place a child at risk. The Academy follows Sport Integrity guidance on photographing children and young people. sportintegrity.gov.au
8. Social Media & Communications Policy
8.1 Parents, guardians and Participants must use social media responsibly and respectfully in relation to the Academy, its staff, volunteers, other participants and opposing clubs. Abuse, harassment, defamatory comments, or targeted campaigns against individuals will not be tolerated.
8.2 Direct messaging of coaches by participants (minors) should be via a parent/guardian or through official channels; coaches should use appropriate public channels for general team communications. The Academy will set clear guidelines about coach-player communications on electronic platforms.

9. Code of Conduct & Disciplinary Actions
9.1 The Participant and Parent/Guardian agree to comply with the Academy’s Code of Conduct and all relevant policies (Member Protection, Child Safe, Anti-Bullying).
9.2 Breach of the Agreement or Code may result in disciplinary measures including warnings, suspension from activities, expulsion from the program, or other actions determined by the Academy. Serious breaches will be escalated according to the Academy’s policies and relevant governing bodies.

10. Complaints & Dispute Resolution
10.1 Concerns should be raised initially with the Participant’s coach or the Academy administration. If unresolved, the Academy will follow its internal complaints procedure.
11. Governing Law
This Agreement is governed by the laws of South Australia, Australia, and any disputes will be dealt with in that jurisdiction.
